You will see that some fields are compulsory. For other fields please do your best to complete the information requested and upload all relevant documents as this will greatly help David to understand what is needed and to avoid unnecessary additional costs to you.

David will coordinate the date, time and place of your meeting with him. However if there is a preferred date please select that below.

You may have received a communication (e.g. email, letter, etc) advising you that you need to see a Notary Public. If so, please upload a copy of that communication. If it is an email, please forward a copy of that email to david@norlaw.co.nz

Please select the notarial service or services you require. If more than one, please select all that you require.

For each selection you make, further information will drop down and be available for entry data and document uploading at the bottom of the selection list. Click on the Add Entry link for as many selections as you have and enter the information requested in the new window that appears.

Please also complete the Additional Information section. The Any other details David should know? field can be used for any information you think may be relevant or to just give him details which you have not been sure are covered in the rest of the form.

Note that for Power of Attorney notarisation, David would only be providing notarial confirmation of the execution of the document itself and not that the Power of Attorney is valid, effective or appropriate.

Although you are uploading copies of the documents which require notarisation, David will need to see the originals when you meet with him.
